Functional Rhinoplasty for Rhinitis: Improving Both Nasal Breathing and Aesthetic Nose Design

The nose plays a vital role in both breathing and sense of smell, while also serving as the central feature of the face that defines overall balance and dimension. Because of this, the functional and aesthetic aspects of the nose are closely interconnected. When either function or appearance is compromised, comprehensive improvement may be necessary.
Common functional nasal issues include deviated septum (septal deviation), chronic rhinitis, sinusitis, and nasal valve collapse, all of which can lead to nasal congestion and breathing difficulties. In many cases, patients with these conditions also present with visible structural or cosmetic concerns.
For example, a deviated septum occurs when the cartilage inside the nose is bent, which may also result in a visibly crooked nose. Similarly, nasal valve collapse often appears as a drooping or flattened nasal tip, affecting both airflow and appearance.
To address both concerns simultaneously, a specialized procedure known as functional rhinoplasty for rhinitis—or functional + cosmetic rhinoplasty—has gained attention. Unlike standard cosmetic nose surgery, this approach improves nasal function and aesthetics together, enhancing breathing while creating a more balanced and refined nose shape.
A key step in this procedure is the accurate diagnosis of internal nasal structures. Evaluating only the external appearance is not sufficient. Advanced diagnostic tools such as 3D CT scans and nasal endoscopy are used to assess the nasal bones, septal cartilage, inferior turbinates, and sinuses. This allows surgeons to identify the exact cause of functional issues before establishing a surgical plan.
Once the internal structure is fully analyzed, the underlying causes—such as septal deviation, nasal valve collapse, or hypertrophic rhinitis—can be addressed accordingly. At the same time, aesthetic planning is carried out to create a harmonious nose shape. This includes designing a “dual-line structure”, which enhances both the external nasal bridge and the internal support framework for long-term stability.
In particular, building a strong internal nasal structure alongside the external nasal line is essential in Korean rhinoplasty techniques. This ensures that the nose maintains its shape over time without deformation. By first correcting functional problems and then refining the external appearance into a natural yet refined (“natural-glam”) nose, patients can achieve both improved breathing and a modern, aesthetically pleasing result.
